Acute-on-chronic liver failure (ACLF) is a severe form of chronic liver disease associated with multi-system organ dysfunction and high short-term mortality. High-volume plasma exchange (PLEX) is one of the therapeutic measures that improves prognosis. We present the case of a 32-year-old man with alcohol-related liver disease who was admitted with acute decompensation, including coagulopathy, hepatic encephalopathy, and acute kidney injury precipitated by alcoholic hepatitis.

Background Obese patients are at an increased risk of obstructive sleep apnea (OSA). Bariatric surgery or weight loss surgery is an important therapeutic measure in obese patients for the management of weight and comorbidities. Data are scarce in inner-city Hispanic and Black patients who undergo bariatric surgery, which eventually leads to health disparity in this minority population.

Background: Paraesophageal hernia repairs (PEHRs) have high rates of radiographic recurrence, with some patients requiring repeat operation. This study characterizes patients who underwent PEHR to identify the factors associated with postoperative symptom improvement and radiographic recurrence. We furthermore use propensity score matching to compare patients undergoing initial and reoperative PEHR to identify the factors predictive of recurrence or need for reoperation.
