Methods of reconstructive maxillofacial operations bringing about the best functional and esthetic results are analyzed. Surgical methods for repair of maxillofacial bone defects and deformations with the use of transplants and different methods of their fixation depending on the tissues at the site of defect (deformation) in patients with acquired and congenital abnormalities of the facial skull have been developed and are used at Central Institute of Dentistry. Osteosynthesis using titanium constructions and compression-distraction devices is acknowledged as an effective method of bone repair operations on the face.

Intraoral supraosseous compression-distraction devices (CDA) have been used for repair of mandibular underdevelopment and defects of different etiology at Central Institute of Stomatology since 1997. The distraction regenerates have been examined by ultrasonography, roentgenography, and echo-osteometry. These methods allow timely correction of the course of distraction and rule out formation of wrong hypotrophic distraction regenerates.

We have treated 33 young men with medium to large (3-8 cm) bony and soft tissue defects of the lower third of the face caused by gunshot wounds. After debridement, collapsing the proximal segments for primary approximation of soft and hard tissues and a closed osteotomy of a small fragment of mandible, we used an original compression-distraction device, designed in 1982 and tested during 1983 (analogous devices were absent at that time) to reposition the mandible and cause callus to form (during distraction) between the fragment and to use the remaining stumps of bone to fill in the defect. The soft tissues were repaired at the same time.
