Introduction: Managing choledocholithiasis alongside cholelithiasis has seen significant evolution, presenting options such as single-stage cholecystectomy with common bile duct (CBD) exploration, or a two-stage procedure involving endoscopic retrograde cholangiopancreatography (ERCP) performed before or after cholecystectomy. Evidence from Western settings may not fully capture the unique disease burden and demographic characteristics prevalent in developing countries. Laparoscopic common bile duct exploration (LCBDE) is more commonly performed in the Indian subcontinent compared to Western centers due to limited access to advanced procedures and specialized instrumentation for complex ERCP cases.
View Article and Find Full Text PDFsymmetric chiral 7,12-dioxa[8]helicenes were synthesized through a series of photochemical - isomerization, electrocyclic reaction, and oxidation steps in a stepwise sequential manner at both the ends of 2,9-di(()-styryl)naphtho[2,1-b:7,8-b']difuran. The chemical transformations complemented with density functional theory (DFT) studies delineate some fundamental concepts, exhibiting counter current effects, namely, destabilization caused by increasing steric crowding and stabilization caused by aromatic units on the overall transformation. The calculated energy profile diagram unravels the formation of photoinduced intermediate species with increasing free energies for the - isomerization and the electrocyclic reactions; the reverse processes for the said steps are prevented by a specific barrier-less oxidation step forming aromatic rings, presenting a one-way valve situation.

Uncommon complications post-hepaticojejunostomy demand scrupulous attention, and this case report reveals a seldom-documented scenario of jejunal angiodysplasia as an elusive cause of postoperative bleeding. Through a comprehensive examination of the patient's clinical trajectory, diagnostic challenges, and subsequent management, this report contributes to the expanding knowledge base on atypical vascular complications in surgical settings.

Concurrently, addressing ventral hernias in morbidly obese patients undergoing bariatric surgery presents a significant surgical challenge, amplified by the larger intraperitoneal cavities and the presence of large hernial sacs. This technical report introduces two innovative techniques to alleviate the challenge of hernia sac distension due to pneumoperitoneum associated with simultaneous bariatric surgery and ventral hernia repair using laparoscopic technique.
