Background: Biological medications have played a significant role in maintenance therapy for Crohn's disease (CD), but some cases become refractory to these agents. Methotrexate (MTX) appears to be a cost-effective and readily available drug for enhancing the effectiveness of maintenance therapy when used in combination with anti-tumor necrosis factor (anti-TNF) therapy in such cases. However, its effectiveness is still to be established.

Article Synopsis
  • This study examined how patient-reported disease burden and health-related quality of life (HRQoL) affect the decision to switch from nonbiologic to biologic therapy in people with plaque psoriasis.
  • It included biologic-naive patients aged 18 and older who were using nonbiologic treatments and gathered data on their disease burden and quality of life when they entered the CorEvitas Psoriasis Registry from April 2015 to August 2022.
  • Results showed that a higher reported burden was linked to a greater likelihood of switching to biologic treatment, indicating that patient experiences significantly influence treatment decisions alongside clinical assessments.

Background: The International Psoriasis Council (IPC) updated the classification of psoriasis severity to guide clinical decision-making. According to IPC guidelines, patients are considered candidates for systemic therapy when body surface area (BSA) is >10%, when lesions affect special body areas (ie, face, palms, soles, genitals, scalp, or nails), or when topical therapy fails to control symptoms.

Objective: To evaluate patient candidacy for systemic therapy in real-world settings, according to disease severity criteria.

Article Synopsis
  • Minimally invasive spinal surgery techniques like oblique lateral lumbar interbody fusion (OLLIF) provide better outcomes but involve dual C-Arm imaging, which hasn't been fully studied for radiation exposure to surgeons.
  • A study measured radiation doses during 121 OLLIF surgeries over 12 months, finding an average exposure of 0.06 mSv per surgery, well below safety limits.
  • The results emphasize the need for protective measures and monitoring radiation exposure, potentially leading to new guidelines in surgical practice. Future research could explore larger surgeon cohorts and more complex procedures.

Appendiceal mucocele is a rare entity first described by Carl von Rokitansky, characterized by cystic dilatation of the appendiceal lumen due to obstruction, epithelial proliferation or inflammation and accumulation of mucoid material. The cause can be either neoplastic or non-neoplastic. Patients with appendiceal mucocele can be asymptomatic or present with right lower quadrant pain which may mimic acute appendicitis.
