In 411 patients with acute myocardial infarction (AMI) the effect of intravenous lidocaineeee administration was studied in a suboptimal dose - under 1,5 mg/min (to 1979) and in the optimal dose - over 1,5 mg/min, minimum 2 mg/min during the first 12 hours, with an average 24 h dose, 3,0 g in the course of 48 hours. The patients were grouped into three prognostic groups, depending on their initial assessment according to the coronaryprognostic index of M. R.

Cardiac output was determined to 70 patients with hypertonic disease I, II and III stage by dye-dilution method and urine excretion of adrenaline and noradrenaline was determined by fluorescence method. The extent of stroke and minute volume was determined depending on the stage of the disease, degree of arterial hypertension and patients' age. Noradrenaline excretion was normal in 75.

The stage of cardiomegaly, the stasis in the pulmonary and systemic circulations were followed up as well as the presence of manifestations of pulse deficit, incidence of gallop rhythm manifestation. It was established that in the patients with II-A stage of decompensation of circulation, a decreased cardiac output was found, enhanced peripheral resistance and normovolemia.

The incidence, intensity and duration of stenocardiac paroxysms were followed up as well as the nitroglycerine need- and electrocardiographic changes. The main hemodynamic indices, prior to and post treatment, were also determined.

The patients were grouped into three groups depending on the degree of pulmonary--capillary pressure increase (PC)--greater than 2.666, greater than 3.

The motor programme was used for the treatment of 241 patients, distributed into 2 groups. Observation on the effect of peak physical loading at the end of each motor regime was also reported.

Some of the patients were examined by removing the detector, aiming at magnifying of the image and enhancing the differentiation ability under I cm. In some borderline cases, the patients were examined under physical loading and at rest for the differentiation of the normal from the disperfusion myocardial zones.

The results obtained revealed that in 76 per cent of the cases with ventricular extrasystoles, the treatment result was very good, with complete disappearing of extrasystoles. Good effect was found in 18 per cent, with the recording of some rare, single extrasystoles towards the end of the treatment, and the treatment had no effect on 6 per cent of the patients.

Cardiac capacity and volume of circulating blood were determined in 42 patients with atherosclerotic myocardiosclerosis and acquired valvular defects. A very high correlation dependence was established between the two methods.

At necropsy, two thrombi were found, one of them "traveling", situated very close to each other, providing conditions for a complete obstruction of the mitral opening and imitating the respective clinical picture.

A decreased tolerance to physical loading was established: cardiac index--insignificantly increased, beat volume-decreased, volume of circulating plasma and blood--tendency to increase. The results show a dependence on the clinical development of the disease--the signs of a latent insufficiency being manifested in the cases with extensive localization of the infarction, transmural affection and with a formed aneurisma after a dosed loading.

The alveolar-capillary gas metabolism was studied in 60 patients with acute myocardial infarction. Arterial hypoxemia and hypocapnia were observed in the majority of the patients. Those disturbances are more pronounced in the presence of cardiac insufficiency.

Manifested changes in the hemodynamics were established, namly: pulse rate and cardiac flow decrease, the total peripheral resistance grows; veloqcilty of blood flow is slowed down. The preparations used have a negative effect upon the phase structure of left ventricle as well.
